Compliance with Bank of Namibia Act 15 of 1997: Credit Bureau Regulations

I herewith acknowledge and understand that:

1.⁠ ⁠TwentyTwenty Financial Solutions (Pty) Ltd will be entitled to make enquiries about my payment profile and credit performance history for the purposes of compiling a credit assessment as provided for in the Bank of Namibia Act No. 15 of 1997: Credit Bureau Regulations (Regulation 15 and 16);
2.⁠ ⁠The assessment will relate to both favourable and unfavourable credit performance information (Regulation 15 and 16);
3.⁠ ⁠TwentyTwenty Financial Solutions (Pty) Ltd will be obligated to provide such credit performance information to all credit bureaus and credit providers which have subscribed to receive credit performance information from credit bureaus and such information will include both favourable and unfavourable information (Regulation 22);
4.⁠ ⁠Without any notice to me, TwentyTwenty Financial Solutions (Pty) Ltd will be obligated and entitled to inform credit bureaus in the event that I default on any loan agreement that is provided to me by Twenty Twenty Financial Solutions (Pty) Ltd, fail to effect payment or make late payment in terms of such loan agreement (Regulation 14(2)); and
5.⁠ ⁠I have satisfied myself of the meaning and consequences of the above-mentioned.

Consent to disclose personal information

I herewith consent to the disclosure of my personal information, listed below, (“Information”) by my employer,
or by me directly to Twenty Twenty Financial Solutions (Pty) Ltd, its order, successors in title or assigns, (“20Twenty”) for the purpose of attending to an assessment for the purpose providing and/or originating financial services and/or products, including but not limited to mortgage loans, and for any other purpose relating thereto, to me.

The following information and/or copies of documents may be disclosed, namely: -
1.⁠ ⁠Identity Document;
2.⁠ ⁠Marital Status;
3.⁠ ⁠Salary and/or income information;
4.⁠ ⁠Any change in salary and/or income;
5.⁠ ⁠Period of uninterrupted employment with Employer;
